To request a repair of your device please submit this form and provide as much detailed information. Once received someone will contact you within 2 business days with followup questions and instructions for shipping device in for repair.
If device is physically damaged please submit pictures of the device that include Front, Back, battery compartment, Serial Number, Warranty Tags and any damage to device. Send pictures via email to email@example.com. When shipping device include battery only if requested and remove any password if applicable, to allow proper testing.
If device is still under warranty, coverage will be determined once the device is received and inspected.
